Dr. Feldman provides comprehensive foot and ankle care.  His training encompasses foot and ankle surgery, biomechanics (orthotics), sports medicine and general podiatry.  As noted in his biography (HOME) and in LECTURES AND PUBLICATIONS he is a leader in surgery and sports medicine.  He is also a leader NEW MEDICAL TECHNOLOGIES always striving to improve patient outcomes and speeding patient rehabilitation.

SURGERY

In most instances conservative, non-surgical care will alleviate the majority of foot problems.  However, when the patient fails to respond to conservative care or there is a fixed structural deformity, surgery oftentimes becomes necessary.  It is the patient's decision to proceed with surgery.  Dr. Feldman specializes in the most common foot surgeries including but not limited to; hammertoe repair (straightening crooked toes), bunionectomies, neuroma excision and repair of arthritic joints.  He takes pride in using advanced surgical techniques for the treatment of painful flat feet, tendon injuries and painful unstable metatarsophalangeal joints (the ball of the foot).  Dr. Feldman uses a combination of local anesthetic, anti-inflammatories and pain medications to maximize postoperative pain management.

Dr. Feldman is Board Certified in Foot Surgery by the American Board of Podiatric Surgery.  He has regularly scheduled operating room time at the Sharp Hospital Out-Patient Pavilion Surgery Center and the OASIS Surgery Center.

SPORTS MEDICINE

The essence of sports medicine is found in two principles: integration and rehabilitation.  Dr. Feldman's goal is to integrate care with athletic trainers, coaches, personal trainers and physical therapists.  Integration also refers to addressing strength, balance, speed and flexibility.  Rapid rehabilitation and return to activity is important.  The principles of sports medicine apply to competitive athletes, recreational athletes, injured workers, surgical patients and patients both young and old.  If it is your goal to get better......why not get better faster!

Dr. Feldman is an expert in foot orthotics.  He provides orthotics to the San Diego Chargers and the Athletic Medicine Departments of San Diego State University, University of San Diego, Point Loma Nazarene University and Christian Heritage College.  While he takes pride in treating such high level athletes, Dr. Feldman has prescribed far more orthotics to recreational athletes and non-athletes.  Orthotics are an important conservative care modality and are a part of many treatment regimens.  Orthotics are effective in treating plantar fasciitis, arch pain, general foot pain and fatigue, metatarsalgia, ankle instability, tendinitis, shin splints and flat feet.

TENDON REGENERATION

The most common lower extremity tendon injuries include: Achilles tendinitis, posterior tibial tendinitis and peroneal tendinitis.  The plantar fascia is technically a ligament, but has the microscopic structure of a tendon.  Tendon inflammation (tendinitis) is treated with medication, rest, support, stretch, orthotics and physical therapy.  Oftentimes, the tendon injury becomes more chronic and the tendon becomes degenerative (tendinosis).  With tendinosis the tendon tissue becomes thick and avascular (without adequate circulation).  The tissue quality degrades and the body is unable to heal the tendon.  If the tendon becomes torn, surgery may be indicated. With tendinosis and/or micro-tears, new technologies including extracorporeal shock wave therapy and platelet rich plasma may be indicated.  Both of these technologies stimulate tendon healing through the release of tissue growth factors.  Tendon circulation and collagen quality improve.  The tendon then has the ability to heal.  Dr. Feldman has been an instructor for extracorporeal shock wave therapy and has been a medical research site for platelet rich plasma studies.
